Clarification of Health Screening Process 2005
Non-fire funded personnel at SO

If you participate on fires in an ARDUOUS position:
(example..DIVS, STEN, CRWB, etc.)

1. You must be registered in the incident medical standards database. “FFMO” does that.
2. You must have a conversation with “FFMO” about your participation in this process and fire participation/availability.
3. The medical provider will contact you with an appointment (you have to go to a specific contracted provider)
4. Your results go to a medical review board to determine if you are cleared for duty or pending a further exam

If you participate on fires in a MODERATE or LIGHT position:
(example SOF2, BAER TEAM, etc.)

1. You must go to Human Resources and get a Health Screening Questionnaire (HSQ)
2. Fill out HSQ and return it to HR, they will determine if you need to take a physical
3. HR will call FFMO to confirm your participation in fire
4. When confirmed, HR will give you a packet to take to the Dr.
5. You may go to the Dr. of your choice.
6. The results go back to HR
7. The bill goes to Lead Dispatcher
